How to Change Your Name

Need to Change Your Name?

 Process for changing your name:
  1. Change your name with the Social Security Administration Office. 
    (Find a Branch near you)

  2.  Once you have received your new card bring it to:

    Employment Services
    5700 Cass Ave
    Suite 1900
    Detroit, MI 48202
     
  3. You will be instructed to fill out a name change form.
    (Name Change Form) 

  4. You will need to complete a new I-9. Please review the acceptable documentation that you can bring to complete the I-9
