This checklist is designed to help you make your initial benefit enrollment selections in a timely manner. As a new employee, please note that there are certain deadlines that must be met to expedite your benefit plan enrollments. Generally, you have 30 days to choose the benefits most useful to you and your family members. Please make your benefits elections carefully.
All benefit plans administered by the Total Compensation and Wellness department are listed here. You may not be eligible for all benefit plans listed, please reference your union contract or employee manual for details.
You can monitor your enrollment and ongoing benefit activity via your electronic Benefit Statement and Pay Stub on Pipeline. Please contact the Total Compensation and Wellness department if you have any questions relative to your benefits and compensation package.
MEDICAL INSURANCE
Deadline for Enrollment Forms: 30 days from date of hire. If you do not enroll within 30 days, you will not have insurance coverage.
Effective Date: First of the month following or concurrent with your date of hire. PLEASE NOTE: Your Medical Benefit Deductions are prepaid and may include retroactive employee contributions that were due prior to the effective date of coverage. See “Paying For Your Medical Benefits” for more information.

Failure to Enroll in Medical Insurance within 30 Days
If you fail to enroll in medical insurance within the 30-day deadline, you will be defaulted to no medical insurance coverage. You will not be entitled to the cash-in-lieu of medical benefit or (if you are in an eligible employee group) vision coverage. You will be unable to enroll in a health plan until the next Open Enrollment period, usually in October, with coverage effective January 1st of the following year.
Generally, there are three times when you can enroll in or may be able to change your benefits at the University:
• as a newly hired or newly eligible faculty or staff member,
• after experiencing a qualified family status change, and
• during Open Enrollment.
CASH IN LIEU OF MEDICAL INSURANCE
Deadline for Enrollment Forms: 30 days from date of hire. If you do not enroll within 30 days, you will not receive this benefit. Please note: This benefit is not retroactive.

DENTAL INSURANCE

LIFE INSURANCE/DEPENDENT LIFE
Deadline for Enrollment Forms: 30 days from date of hire. If you do not enroll within 30 days, you will be given basic coverage. The default beneficiary designation is "My Estate". Enrollment for supplemental coverage after the deadline is subject to the approval of The Hartford. You will have to complete and submit a personal health application for review and determination of eligibility.
Effective Date: Date of hire.

VISION PLAN
For non-represented employees and employees represented by the AAUP-AFT, P&A Local 1979, HERE Local 24, Operating Engineers Local 547, Operating Engineers Supervisors Local 547, and AFSCME Local 1497. Enrollment is bundled with the medical plan that you select. There is no form to enroll in the regular vision plan.
If you have chosen the Cash-in-Lieu of medical insurance benefit, you may enroll in the Voluntary Vision Plan within 30 days of your date of hire. Please use the form below to enroll in the voluntary vision plan. Coverage begins on the first of the month following or concurrent with your date of hire.

RETIREMENT PLAN
Participation in the retirement plan is voluntary, as such, there is no deadline for enrollment. Eligible employees can enroll in the plan regardless of age or length of university service. Eligibility for matching employer contributions is subject to a two-year waiting period which may be waived with appropriate prior qualifying service. University contributions are not retroactive.

TUITION ASSISTANCE PROGRAMS
Eligibility Deadline: Eligible employees must have a current hire date that is on or before the last day of Open Registration to qualify for the subject term.
